I took my husband's last name and made my maiden name my middle name.
I used a free name change website that my cousin sent me -- http://newlastname.org. I found it very helpful. Got all the forms and everything ready in less than 30 mins.
If you are going to change your name, I would recommend going the free route rather than using a pay service. They don't add much from what I've heard.
